ASSEMBLYMEMBER KAY’S STATEMENT ON $10 MILLION CAPITAL GRANT FOR THE TRANSITIONAL LIVING COMMUNITY AT TCFD

ALBANY, NY — The FY27 State budget includes a $10 Million Capital Funding Grant for The Center for Discovery to construct their Transitional Living Community. This transformative project will directly address one of New York’s most pressing healthcare challenges: ensuring access to high-quality, advanced medical care for aging adults with complex disabilities and medical fragility and will be located right in the heart of Assembly District 100.
The goal of the Transitional Living Community is to provide small, highly supportive medical care neighborhoods for forty aging adults to reduce the frequency and duration of acute hospitalizations, prevent unnecessary discharges to traditional nursing facilities and promote stability, dignity, and quality of life for residents, while giving families peace of mind. This investment will also yield savings for the State by avoiding prolonged hospitalizations and inappropriate nursing home placements, reduce pressure on hospitals ensuring safer transitions for medically complex patients out of acute care, advance care coordination across the health care continuum, strengthen workforce expertise in complex care, and create 200 new jobs in Sullivan County.

Their work pushes the limits and redefines areas of care for individuals with developmental disabilities and this new project is a testament to their consistency and their compassion. This facility will save many lives and will serve as a shining example for health care agencies all around the state to follow.
