Advancing Care for Aging Adults with Complex Disabilities
At The Center for Discovery, we are developing the Transitional Living Community (TLC) to address a critical gap in care for aging adults with intellectual and developmental disabilities (IDD) and medical frailty.
As individuals with complex disabilities live longer, many require a level of medical support that traditional residential settings are not designed to provide. Too often, this leads to repeated hospitalizations, emergency interventions, and transitions into settings that cannot fully meet their needs.
The TLC will operate as an Article 28 Skilled Nursing Facility demonstration project on TCFD’s Rock Hill campus, providing advanced, coordinated care in a setting designed specifically for clinical complexity and quality of life.
The goal is to reduce avoidable hospitalizations and improve quality of life by providing compassionate, specialized care in an environment grounded in dignity, continuity, and human connection.
This investment represents hope and peace of mind for families who have too often struggled to find the right level of care for aging loved ones with complex disabilities. As this population lives longer, a critical gap remains in the healthcare system for individuals whose needs are too medically complex for traditional facilities. We are deeply grateful to Governor Hochul, Assemblymember Kay, and our agency partners for supporting a more proactive and coordinated model of care focused on improving quality of life, reducing unnecessary hospitalizations, and helping individuals live with greater stability and dignity across their lifespan. Through this demonstration project, New York continues to lead the way in healthcare innovation and the advancement of complex care.
